Session Agreement

Please read through and complete the policies on this page at least one week before your portrait session. By checking each box and typing your name at the bottom, you are confirming that you have read, understand and agree to the session agreement/contract.

Name(required)

The session fee includes any pre-session consultation via email or telephone, one or two hours of coverage on your scheduled date, post-session image processing and editing and an online gallery for proofing and ordering your portraits.(required)

To reserve your session date, session fees must be received within seven days of scheduling your session.

Session fees are non-refundable but may be applied to a rescheduled session with a 24 hour cancellation notice.(required)

Payment Options Include Cash or Check ($35 fee for returned checks)(required)

All images are protected by federal copyright laws and may not be copied, scanned, downloaded or reproduced in any way without written permission from the photographer.

hereby consent that photographs taken by Jenifer Golen may be reproduced in both printed and onine form by Jennifer Golen for the purpose of display, portfolio, advertising, website to promote her studio and name. I also agree that these images may be used without additional compensation to the undersigned. I have read this model release and fully understand the contents thereof. I confirm that I have legal authority to grant these permissions for myself and the included minor children. (Please fill in the full name and birthdate of each person for whom this model release will be valid.)(required)
